Community Health Pharmacy
Patients who have a Primary Care Physician at certain qualified health centers including CHSLV may be eligible for discount pricing on prescriptions through Community Health Pharmacy. Patients can contact Community Health Pharmacy directly at 802-893-1120. If they need further assistance they can contact a member of the Community Health Team to get set up.

The Medication-Assisted Treatment [MAT] Team specializes in opioid and alcohol use. The MAT Team offers substance use and MAT screening, referrals to treatment, and care coordination at locations across the Lamoille Valley Region.
