Healthcare

Community Health Team:

The Community Health Team is here to support you and your family while navigating the health
care system. The Team helps connect you to available resources and supports that can improve
your health and wellbeing.
Community Health Team services are free to all!
Some examples of what CHT may be able to help you with:

  • Applying for health insurance,
  • Solving transportation challenges,
  • Connecting you with resources for housing,
  • Finding resources for accommodations in your home (ramps, etc.),
  • Registering for free programs such as the Diabetes Prevention Program, Diabetes Self
    Care Online Classes, and Smoking Cessation,
  • Connecting you with resources for finding a job or receiving modifications at work,
  • Diabetes education and resources for supplies,
  • Short term counseling (“brief intervention”) for mental health needs,
  • Eating on a budget and nutrition coaching for any need.

Our team includes care coordinators who are:

  • Registered Nurses
  • Registered Dietitian Nutritionists
  • Certified Diabetes Educators
  • Social Workers

Copley Hospital is located at 528 Washington Highway, Morrisville. They provide Lamoille's hospital needs, including a 24 hour emergency department.

MAT Team

The Medication-Assisted Treatment [MAT] Team specializes in opioid and alcohol use. The MAT Team offers substance use and MAT screening, referrals to treatment, and care coordination at locations across the Lamoille Valley Region. Reduced-Cost Eyeglasses

Community Health Pharmacy

Patients who have a Primary Care Physician at certain qualified health centers including Lamoille Health Partners may be eligible for discount pricing on prescriptions through Community Health Pharmacy.  Patients can contact Community Health Pharmacy directly at 802-893-1120.
